Closing the Continuity-of-Care Gap
Border and migrant populations — especially female workers moving between Special Economic Zones — frequently see care fragmented across facilities and provincial lines, with no shared record to carry their history with them. Output 3 addresses this systemic gap directly: an Electronic Medical Record (EMR) and Unique Health Identifier (UHI) are being rolled out across 8 district hospitals and 3 provincial hospitals in Champasak, Phongsali, and Savannakhet, so every patient’s record travels with them wherever they seek care.

From Software Design to Full Rollout
Output 3 moves from platform design to full district-level rollout over three phases, delivered by the Digital Health Firm in coordination with the PMU’s IT Coordinator.
Q4 2024 — Develop the EMR/UHI Software
The Digital Health Firm designs and builds the EMR platform and the Unique Health Identifier system, tailored to Lao PDR’s border-province hospital context.
Q1 2026 — Install Equipment in Provincial Hospitals
Hardware and system equipment are installed in the 3 provincial hospitals — Champasak, Phongsali, and Savannakhet — establishing the backbone for EMR/UHI rollout.
Q2 2026 — Roll Out EMR in District Hospitals
EMR goes live across the 8 district hospitals, giving every patient a Unique Health ID that follows them across facilities and provincial lines.

Electronic Medical Records (EMR)
Standardized digital patient records replace paper charts across the 11 target hospitals, giving clinicians a patient’s diagnoses, treatments, and referral history at the point of care.

Every patient is assigned a single Unique Health Identifier that links their record across every facility they visit — critical for mobile and migrant populations moving across provincial lines for work and care.
• One identifier follows the patient across all 11 hospitals
• Directly addresses the systemic gap of fragmented records and lost care history across provincial lines
• Supports continuity of care for cross-border and internal migrant workers, including women in SEZs
• Designed alongside the EMR platform during the Q4 2024 software development phase

The Digital Health Firm
One of BAHP’s Strategic Consulting Firms, the Digital Health Firm is responsible for software design and EMR/UHI rollout support across all 11 hospitals.

PMU IT Coordinator
A national individual PMU consultant, the IT Coordinator provides day-to-day technical oversight of EMR/UHI delivery, coordinating between the Digital Health Firm, provincial hospitals, and district facilities.
